
I. General information
We want your visit to be comfortable, convenient, and efficient. We have attempted to create both a warm and professional environment that allows this to happen. We know these visits can at times be stressful, and we will work hard to eliminate that as much as possible.
II. What about if I’ve been an established patient elsewhere?
If you were seeing a gynecologist other than Dr. Haley but now wish to transfer your care, we would be delighted to see you! Simply contact your previous doctor’s office and request they send a copy of your records to us, preferably before your actual visit.
Be aware that you have every right to your previous records, but only the patient, not the doctor, has the right to request these.
III. What if this is my first visit?
As you probably understand, first visits take a little longer. We want, and need, to get to know you! Therefore, plan to spend some extra time with us, and try and arrive 15 minutes before your scheduled appointment. We will need to have you fill out the necessary registration and medical forms, verify insurance coverage, and answer any questions you may have. If you were sent a new patient information packet, bringing these completed forms with you will definitely help speed up the process. We are here to serve YOU, but this is some of the necessary paperwork that must be taken care of on a first time visit. If you are taking any medications, please bring a list of them, and the dosages, or else bring the medications themselves with you if that is easier for you.
IV. What if I am going to be late, or am unable to keep my appointment?
Please call us as soon as you are aware of the problem, and we will see how best to accommodate you. But we would greatly appreciate a courtesy call.

VI. What if I do not have health insurance?
We understand that in today's tough economic times, many people are without health insurance. We see many patients that are self-pay, and Dr. Haley prefers his patients come on a regular basis, regardless of their economic situation. Your health is of upmost importance, and we will work with you if you have a financial crisis. He always gives self-paying patients a 20% discount at the time of visit, or we can work out a payment plan with you. You may pay 1/3 of the visit up front, and pay the rest in two monthly payments. He also is the staff Gynecologist for the Paulding County Health Department if that is an option for you. Please call our office if you have any questions about our self-paying options.
